Community Health’s mission is to provide high quality public health nursing to our member communities. We provide essential services of communicable disease investigation and follow-up, high risk maternal child services and health promotion visits. Our Staff serves as the “Town Nurse” for our member communities.
The primary focus of your Town Nurse is on patient, family and communication as well as crisis intervention and support to high risk individuals.
Community Health conducts clinics (well adult and flu) and public education in our member communities.
