Hospice RN Case Manager - full or part time

As a member of the interdisciplinary team, the Hospice RN works under the direct supervision of the Clinical Manager.  The Hospice RN will coordinate nursing assessment, planning and care in order to maximize the comfort and health of patients and their families. S/he is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the primary care physician’s orders and the plan of care.

Job Requirements:

  • Current RN MA Licensure.
  • Minimum of 2 years of Hospice and Homecare experience required, 2 years medical/surgical, subacute experience preferred.
  • Currently holds a valid driver’s license.
  • Must be flexible to make home care visits as agency needs.
  • Required on call rotation for weekends, evenings and Holidays.
  • Ability to learn and be proficient with required documentation software using agency electronic device.

Homecare and Hospice Social Worker (LICSW) Part-time to Full -time

Assist and counsel patients and families with financial, social and emotional concerns primarily in hospice and homecare programs. 

Job Requirements:

  • Bachelors or Master’s degree from a school of social work accredited by the Council on Social Work Education.
  • Licensed as an independent clinical social worker (LICSW) in MA.
  • Minimum of one to three years of experience in social work, preferably in a home care agency, hospice, hospital, outpatient clinic, rehabilitation center or mental health program.

Clinical Secretary (32-40 hours)

We currently have an opening for a Clinical Secretary to join our team of dedicated and talented professionals serving patients and our member communities.

Responsibilities: Represent the agency in a positive, pleasant, professional manner as initial agency contact by telephone or in person. Greet visitors and vendors issuing visitor badges as needed. Respond to agency telephone calls from public, staff and vendors triaging calls to appropriate staff, taking messages and contacting staff as needed. Perform copying and word processing projects for all sections as assigned. Prepare and processes all mail for delivery to post office, picks up and distributes incoming mail and distributes paychecks. Process and distribute all faxes with date stamp of time received. Organize and assure adequate supply of all agency forms and provider packets. Track, verify and update physician database information on a timely basis. Process the outgoing and incoming physician orders and track the outstanding physician orders as per current procedure in place. Process the discharge summaries as per current procedure in place. Create and files new patient records. File patient information and purges deceased records on a set weekly (or daily) schedule. Organize OASIS documentation for record retention. Track donations, mails acknowledgment cards and complete bereavement cards as needed and assigned. Process patient/family surveys on a predetermined schedule. Enter data into online database system, files completed surveys in a confidential location. Prepare press releases and mailings for Well Adult Clinics, volunteer support training, and bereavement groups. Maintain the inventory and data entry for agency immunizations including flu vaccine both state provided and purchased as well as PPD. Track and fax reports to MDPH as required. Perform data entry and telephone calls for public health as assigned and directed in collaboration with the Community Health Manager. Provide support for Well Adult Clinics, Health Fairs as assigned and directed by Office Manager in collaboration with the Community Health Manager. Provide coverage for Environmental Support staff at special request of the Director of Public Health in collaboration with the Office Manager.


  • High School graduate or Equivalent.
  • Minimum of two years of general office experience which includes reception and secretarial duties.
  • Clarity of voice, pleasant, professional tone and excellent communication and speaking skills.
  • Experience in Microsoft Word with Excel, Publisher or Power Point, etc.
  • Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages, especially the geriatric population.

Licensed Practical Nurse (LPN) – Full-time

Job Summary: The Licensed Practical Nurse performs selected nursing care under the direction of the registered nurse and general supervision of the client care manager.

Job Responsibilities:

  • Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies.
  • Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case.
  • Assists clients in learning appropriate self-care techniques.
  • Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.
  • Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
  • Assists the physician and registered nurse in performing specialized procedures for which competency has been established.
  • Prepares equipment and materials for treatments observing aseptic technique as required.
  • Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
  • Attends client care conferences, meets with the quality improvement reviewer weekly and communicates regularly with the registered nurse.
  • Communicates current health status and needs of clients to appropriate health professionals.

Requirements: Education and Experience:

  • Successful completion of a certified LPN nursing program.
  • Licensed to practice as a practical/vocational nurse in the state.
  • Minimum of one year clinical experience.  Experience in Homecare preferred.
  • Driver’s License

Skills and Abilities:

  • Must currently hold a valid Driver’s License.
  • Must be flexible to make home care visits as agency needs.
  • Excellent communication skills in both verbal and written
  • Excellent time management and interpersonal skills
  • Must be detail-oriented and a team-player

On-Call Home Health/Hospice Registered Nurse

Nashoba Nursing Service & Hospice (NNS&H) is a certified Home Health and Hospice agency under the auspices of the Nashoba Associated Boards of Health. We currently have an opening for an On-Call Homecare/Hospice Registered Nurse. Weekly schedule will be Monday through Friday from 4:30pm to 8:00am. The evenings to be covered are Monday night through Friday night ending at 8:00am Saturday morning.

The right candidate will provide on-call services for Agency home health and hospice patients as received through the answering service. Triage calls and determine if visit is needed to be made after discussion with patient or patient family. Report to scheduled evening/night visits or admissions. Keep ongoing log (as provided by Hospice Director) of all patient telephone calls and visits made during on call from 4:30pm to 8:00am.

This is considered a weekly position and will be paid at a base weekly rate (minimum $385.00/week) plus differential for triaging calls and/or making home visits.
