Marketing/ Community Liaison
We currently have an opening for a Marketing/Community Liaison professional to perform activities associated with customer relations and marketing of the Agency to establish solid referral sources and drive home care and hospice admissions in our service area.
Promote and build strong customer relations, primarily with physician groups including cold calling and setting appointments to explain new and on-going programs to obtain new business.
Develop, implement and monitor sales and marketing plans and action programs to increase and maintain current relationships and referrals and secures feedback from referral sources.
Liaison to hospitals, rehab facilities, skilled nursing and assisted living facilities, physician offices, clinics, etc
Coordinate, plan and prepare the transition of patient care to the home from the referral source.
Plan, direct and develop communication of agency accomplishments and agendas. Publish newsletter regularly for the public and maintain accuracy of mailing lists. Develop advertising materials and ensures NNS&H is visible in the community.
Develop and execute social media strategy and maintain agency website.
Bachelor’s degree with emphasis in marketing with two years relevant sales, marketing communications or public relations experience. Two to three years’ experience in health care or clinical experience as RN, LPN or MSW preferred.
Excellent written and verbal communication skills. Computer experience with various word processing, database, spreadsheet applications and other graphic and presentation software is necessary.
Able to overcome resistance and objections by referrals sources.
Proven track record in producing new business.
Ability to attend health fairs, clinics, meetings with facilities, etc. outside normal workday, for example, Saturdays or evenings.
Compensation: Compensation based on experience and qualifications.

Physical Therapist- Per Visit
We are seeking Registered Physical Therapists with home health experience to provide admissions, evaluations and revisits to patients on our service on an as needed basis. We have availability on weekdays and weekends.
Current RPT MA licensure
Current Driver’s license
Must have 1-2 years of rehab experience in a home health or sub-acute care.
Must be able to work minimum of 8 hours per month and work one major holiday and occasional weekend on-call.
Per visit rate available as well as special rate for weekends.
Electronic documentation experience strongly preferred with the ability to become proficient with a handheld device for home health documentation.
Must be able to work with primarily a geriatric population.
Hospice & Homecare RN Weekday On-Call
Provides hospice and homecare triage of after-hours calls and nursing visits to patients on our service.
Various hours availableUp to 60 hours on-call
M-F 8:00 pm to 8:00 am
Paid on-call rate plus premium for visits

Requirements for above nursing positions:
- Requires Holiday rotation
- Current RN MA licensure
- Current Driver's license
- Minimum of one year home care experience required as well as one year Hospice experience required.
- Must be able to be to make home care and hospice visits as agency needs.
- Ability to learn required documentation software using agency electronic device.
- Must work well within a team.
