Occupational Therapist - Per-diem/Per-Visit

Job Summary: Provide skilled physical therapy and occupational therapy in accordance with the physician’s plan of care. Provide a one-on-one patient experience, treating patients to develop and restore function and maintain maximum performance using appropriate physical therapy and occupational therapy treatments, modalities and procedures as stated in the physical therapy and occupational therapy plan of care.

Job Requirements:

Current PT or OT MA Licensure

1 to 2 years of Homecare experience preferred

Must currently hold a valid Driver’s License

Excellent communication verbal and written skills.

Excellent time management and interpersonal skills

Must be detail-oriented and a team-player

Ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

Registered Dietician – Per-diem/Per-Visit

Job Summary:  Provides home visits for nutritional assessment consultation and medical nutrition therapy under the care of a physician. Provides patient and family education regarding nutrition for wellness and in disease management.

Job Requirements:  BS in related field. Current licensure as registered dietitian in the state of Massachusetts.  Five or more years of professional experience in nutrition. Minimum of one year of health experience. Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages, especially the geriatric population.

Home Health Speech Language Pathologist – Per-Diem

Job Summary:  Evaluates plans and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, and language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.

Job Requirements:  A person who has a master’s or doctoral degree in speech language pathology, and is licensed as a speech-language pathologist by the state of MA. One plus years of experience in Home Care preferred.

to three years of experience in social work, preferably in a home care agency, hospice, hospital, outpatient clinic, rehabilitation center or mental health program.

Certified Nursing Assistant/Home Health Aide – 16 to 24 hours/week

Job Summary:  Provide personal care services such as • reading and recording temperature, pulse and respiration • maintaining a clean, safe and healthy environment •bathing • caring for nails and skin • oral hygiene • assists with self-administered medications • assists with Hoyer lift and other durable medical equipment. Apply safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning and doing household chores. Perform homemaking and other environmental services that facilitate receiving sanitary and safe care at home. Observe, report and document changes in patient status or expectations of service promptly to a supervisor or professional.

Job Requirements:  MA Certified Nursing Assistant Certification is required. One to two years’ experience required. Must have own reliable transportation. Must have valid driver’s license.

Community Health Licensed Practical Nurse – Full-time - Temporary

Job Summary: The Licensed Practical Nurse performs selected nursing care under the direction of the registered nurse and general supervision of the clinical supervisor.

Job Requirements: Successful completion of a certified LPN nursing program. Licensed to practice as a practical/vocational nurse in Massachusetts. Must currently hold a valid Driver’s License. Must be flexible to make home care visits as agency needs. Excellent communication skills both verbal and written. Excellent time management and interpersonal skills. Must be detail-oriented and a team-player. Ability to learn and be proficient with required documentation software using agency electronic device.
