Occupational Therapist - Per-diem/Per-Visit

Job Summary:  Provide skilled physical therapy and occupational therapy in accordance with the physician’s plan of care. Provide a one-on-one patient experience, treating patients to develop and restore function and maintain maximum performance using appropriate physical therapy and occupational therapy treatments, modalities and procedures as stated in the physical therapy and occupational therapy plan of care.

Job Requirements:

Current PT or OT MA Licensure

1 to 2 years of Homecare experience preferred

Must currently hold a valid Driver’s License

Excellent communication verbal and written skills.

Excellent time management and interpersonal skills

Must be detail-oriented and a team-player

Ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

On Call Homecare/Hospice Registered Nurse 

Job Summary:  On-Call Homecare/Hospice Registered Nurse to work evenings, Monday evening through Friday evening from 4:30 pm to 8:00 am Saturday morning. The right candidate will provide: On-call services for Home Health and Hospice patients as the calls are received through the answering service.  Triage calls and determine if a visit is needed to be made after discussion with the patient or patient’s family. Report to scheduled evening/night visits or admissions. Keep an ongoing log (as provided by Hospice Director) of all patient telephone calls and visits made during 4:30 pm to 8:00 am. This is considered a weekly position and will be paid at a base weekly rate (minimum $1,000.00/week) plus differential for making home visits.

Job Requirements:  Current RN MA Licensure, 1 year of either Hospice or Home Health experience is required, valid driver's license, ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

Registered Dietician – Per-diem/Per-visit

Job Summary:  Provides home visits for nutritional assessment consultation and medical nutrition therapy under the care of a physician. Provides patient and family education regarding nutrition for wellness and in disease management.

Job Requirements:  BS in related field. Current licensure as registered dietitian in the state of Massachusetts.  Five or more years of professional experience in nutrition. Minimum of one year of health experience. Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages, especially the geriatric population.

Home Health Speech Language Pathologist – Per-Diem

Job Summary:  Evaluates plans and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, and language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.

Job Requirements:  A person who has a master’s or doctoral degree in speech language pathology, and is licensed as a speech-language pathologist by the state of MA. One plus years of experience in Home Care preferred.

Homecare RN – Full-time (32 to 40 hours per week)

Job Summary:  As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor. The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients, provide professional in-home nursing care to clients, assess and evaluate client's status by reviewing and implementing the plan of care and regularly re-evaluating the client's needs, administer medications and treatments as prescribed by the physician. Observe client for changes in condition and discuss changes with the client, caregiver, and attending physician

Job Requirements:  Current RN MA Licensure, 2 years home health nursing experience preferred (not required), 2 years of medical/surgical, sub-acute preferred (not required), currently holds a valid driver’s license, ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

Certified Nursing Assistant – 16 hours per week and Per-Diem opportunities

Job Summary:  Provides personal care services such as; reading and recording temperature, pulse and respiration, maintaining a clean, safe and healthy environment, bathing, caring for nails and skin, oral hygiene, assists with self-administered medications, assists with Hoyer lift and other durable medical equipment. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning and doing household chores. Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home. Observes, reports and documents change in patient status or expectations of service promptly to a supervisor or professional.

Job Requirements:  Certified Nursing Assistant, one to two years’ experience. Must have own reliable transportation. Must have valid driver’s license.

Volunteer Coordinator – Part-time (20 hours per week)

Job Summary:  As a member of the interdisciplinary team, the Volunteer Coordinator works under the direct supervision of the Hospice and Palliative Care Director. Recruit, screen and train prospective hospice volunteers. Responsible for the scheduling and ongoing supervision including evaluations, competency and proficiency.

Job Requirements:  Associates or Bachelor’s degree preferred or equivalent experience, Previous experience in administrative and supervisory duties, Commitment to the philosophy and goals of the hospice concept, Computer literacy with windows environment, Must be able to work independently, Demonstrated knowledge of appropriate skills for communicating with individuals of all ages, especially the geriatric population, Willing to travel to patients homes, hospital and/or skilled nursing facilities to meet patient needs, Must have reliable and safe transportation and appropriate vehicle insurance coverage and a valid driver's license.

Social Worker, LICSW – Per-diem to cover vacations and medical/personal leaves

Job Summary:  Homecare and Hospice Social Worker (LICSW) to Assist and counsel patients and families with financial, social and emotional concerns primarily in hospice and homecare programs. Provides social work services in accordance with the physician’s plan of treatment. Assists the physician and other members of the home health team to understand the significance of social and emotional factors related to the patient’s care. Assesses and reassesses social and emotional factors in order to help the patient and family to cope with problems in daily living and to help them follow medical recommendations to maximize the patient’s health condition. Counsels patients and families concerning the facilitation of the medical plan of treatment. Recognizes and utilizes community and family resources to assist the patient in long-term plans for life in the community or to learn to live within the patient’s disability. Develops the plan of care with the primary nurse, physician, patient and family to deal with personal, financial and environmental difficulties that may interfere with the health and wellness continuum. Participates in discharge planning. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation within 24 hours of visit.

Job Requirements:  Bachelor’s or Master’s degree from a school of social work accredited by the Council on Social Work Education. *Licensed as an independent clinical social worker (LICSW) in MA* Minimum of one to three years of experience in social work, preferably in a home care agency, hospice, hospital, outpatient clinic, rehabilitation center or mental health program.

Licensed Practical Nurse - Full-time (32 to 40 hours per week)

Job Summary:  The Licensed Practical Nurse performs selected nursing care under the direction of the registered nurse and general supervision of the client care manager. Provide skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies. Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case. Assists clients in learning appropriate self-care techniques. Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals. Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy. Assists the physician and registered nurse in performing specialized procedures for which competency has been established. Prepares equipment and materials for treatments observing aseptic technique as required. Keeps abreast of changes and trends affecting nursing care and licensed nurse functions. Attends client care conferences, meets with the quality improvement reviewer weekly and communicates regularly with the registered nurse. Communicates current health status and needs of clients to appropriate health professionals.

Job Requirements:  Successful completion of a certified LPN nursing program. Licensed to practice as a practical/vocational nurse in the state of Massachusetts. Must currently hold a valid Driver’s License.  Must be flexible to make home care visits as agency needs. Excellent communication skills in both verbal and written. Excellent time management and interpersonal skills. Must be detail-oriented and a team-player.  Ability to learn and be proficient with required documentation software using agency electronic device.  Experience in Homecare preferred.
