Home Care Physical Therapist – full or part-time- Sign on Bonus for full time $2,500

As a member of the interdisciplinary team, the Homecare PT works under the direct supervision of the Clinical Supervisor.  The Homecare PT will coordinate therapy assessment, planning and care in order to maximize the health of patients.

Job Requirements:

  • Current PT MA Licensure.

  • One to two years of experience in physical therapy in a home health setting or rehabilitation facility preferred.

  • Currently holds a valid driver’s license.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekends, evenings and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Physical Therapist Assistant

The Physical Therapist carries out treatment, implementation and documentation of patient status for home health patients under the direction of the registered physical therapist.

Job Requirements:

  • Current MA Licensure as a Physical Therapy Assistant

  • Graduate of a two-year, college-level program approved by the American Physical Therapy Association.

  • Minimum of 2 years of experience as PTA in a hospital, outpatient rehab facility or sub-acute skilled nursing facility preferred.

  • Currently holds a valid driver’s license.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekends, evenings and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Home Care RN Case Manager - full and part time

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Hospice RN Case Manager - full or part time

As a member of the interdisciplinary team, the Hospice RN works under the direct supervision of the Clinical Manager.  The Hospice RN will coordinate nursing assessment, planning and care in order to maximize the comfort and health of patients and their families. S/he is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the primary care physician’s orders and the plan of care.

Job Requirements:

  • Current RN MA Licensure.

  • Minimum of 2 years of Hospice and Homecare experience required, 2 years medical/surgical, subacute experience preferred.

  • Currently holds a valid driver’s license.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekends, evenings and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

LPN / Licensed Practical Nurse / Homecare

The Licensed Practical Nurse performs selected nursing care under the direction of the registered nurse and general supervision of the client care manager.

Job Requirements:

  • Successful completion of a certified LPN nursing program.

  • Licensed to practice as a practical/vocational nurse in the state.

  • Minimum of one year of clinical experience.

  • Experience in Homecare preferred

  • Must currently hold a valid Driver’s License.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekend and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

CNA / Certified Home Care Aides - Full time or part time

Provides support services under the supervision of the appropriate clinical staff that assist the patient or family in the achievement of physical comfort.  Responsible for providing personal care and related household services to clients.

Job Requirements:

  • Current Certification as MA CNA or CHHA.

  • Minimum of one year of related experience.

  • Must currently hold a valid Driver’s License.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekend and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Human Resource Generalist/Recruiter/ Administrative Assistant – Full time

Provides expertise and guidance in all human resource practices, including recruitment, retention, development, communication and implementation of related policy.  Collects, assimilates and interprets personnel data as requested.  Responsible for the overall employee benefits and orientation services of the agency.

Job Requirements:

  • Bachelor’s degree is preferred or equivalent work experience in a related field or HR.

  • Three to five years of human resources experience preferable in home health with at least one year of multilevel supervision and management experience.

  • Solid background with Microsoft Word, Excel, Power Point and Access.

  • Clarity of voice, professional tone and excellent communication and speaking skills.

Hospice Spiritual Care Counselor

Implements and coordinates all activities of the Hospice spiritual care program and the delivery of spiritual services to hospice patients/families.

Job Requirements:

  • Graduate of accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry.

  • Pastoral Counseling Certificate Preferred.

  • Demonstrated ability to be open, sensitive, flexible and ecumenical.

  • Demonstrated experience in working with patients/families dealing with life-threatening illness and death.

  • Demonstrated skill in administrative performance including expertise with both written and oral communication, documentation requirements.
