Position 1 – Home Health Aide – Certified Nursing Assistant

Responsibilities: Provides personal care services such as • reading and recording temperature, pulse and respiration • maintaining a clean, safe and healthy environment •bathing • caring for nails and skin • oral hygiene • assists with self-administered medications • assists with Hoyer lift and other durable medical equipment. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning and doing household chores. Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home. Observes, reports and documents changes in patient status or expectations of service promptly to a supervisor or professional.

Requirements: Current Certification as MA CNA and CHHA (one to two years experience required as a CNA and 2 years experience as a Certified Home Health Aide). Must have own reliable transportation. Must have valid drivers license.

Position 2 – Hospice RN

Responsibilities: Assume primary responsibility for nursing care to patients and support to their families, including assessment, implementation, and evaluation of the plan of care in all settings. Communicates with the Medical Director, other interdisciplinary team members, attending physicians, and other agency personnel for coordination of care and utilization of resources for patient and family. Attends meetings, as a member of the Interdisciplinary Team, and facilitates communication of issues for full staff discussion, consultation, knowledge, and support in updating the plan of care. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family end of life care. Maintains up-to-date electronic patient records so that problems, plans, actions and goals are accurately and clearly stated and that changes are reflected as they occur. Provides appropriate support at time of death and participates in the initial bereavement plan of care.

Requirements: Current RN MA Licensure. Minimum of 2 years of Hospice experience required, 2 years medical/surgical, sub acute experience preferred. Currently holds a valid driver’s license. Must be flexible to make home care visits as agency needs. Required on call rotation for weekends and evenings. Ability to learn and be proficient with required documentation software using agency electronic device. Become a member in the collective bargaining unit Massachusetts Nurses Association.

Position 3 – Home Care RN

Responsibilities: Provides services requiring basic, substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician and makes the initial evaluation visit to the patient. Evaluates, and regularly reevaluates, the nursing needs of the patient. Initiates, develops, implements and makes necessary revisions to the patient’s plan of care. Assesses the patient’s continual care needs. Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs. Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in the patient’s condition. Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Coordinates the total plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients.

Position 4 – PerVisit Physical Therapist and PerVisit Occupational Therapist

Responsibilities: Provides skilled physical therapy or occupational therapy in accordance with the physician’s plan of care. Provides a one-on-one patient experience, treating patients to develop and restore function and maintain maximum performance using appropriate physical therapy or occupational therapy treatments, modalities and procedures as stated in the physical therapy or occupational therapy plan of care.

Requirements: Current PT or OT MA Licensure, 1 to 2 years of Homecare experience preferred.

Position 5 – Physical Therapy Assistant

Responsibilities: Observe, record, and report the patient’s reaction to treatment and any changes in the patient’s condition to the RPT and/or clinical manager. Collaborates with the RPT in assisting the physician in assessing the patient’s functional level by applying appropriate tests. Assists in the development and revision of the plan of care. Treats patients to develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.

Requirements: Current MA Licensure as a Physical Therapy Assistant, Graduate of a two-year, college-level program approved by the American Physical Therapy Association. Minimum of 2 years of experience as PTA in a hospital, outpatient rehab facility or sub-acute skilled nursing facility preferred. Currently holds a valid driver’s license. Must be flexible to make home care visits at agency needs. Required on call rotation for weekends, evenings and holidays. Ability to learn and be proficient with required documentation software using agency electronic device.
