Hospice RN - Per-diem/Per-Visit

Job Summary:  As a member of the interdisciplinary team, the Hospice RN works under the direct supervision of the Hospice and Palliative Care Director. The Hospice RN will coordinate nursing assessment, planning and care in order to maximize the comfort and health of patients and their families. S/he is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the plan of care. Maintains up-to-date electronic patient records.

Job Requirements:  Current RN MA Licensure. Minimum of 2 years of Hospice and Homecare experience required, 2 years medical/surgical, sub-acute experience preferred. Currently holds a valid driver’s license. Must be flexible to make home care visits as agency needs. Required on call rotation for weekends and evenings - Full-time Employee's Ability to learn and be proficient with required documentation software using agency electronic device.

Physical Therapist and Occupational Therapist - Per-diem/Per-Visit

Job Summary:  Provide skilled physical therapy and occupational therapy in accordance with the physician’s plan of care. Provide a one-on-one patient experience, treating patients to develop and restore function and maintain maximum performance using appropriate physical therapy and occupational therapy treatments, modalities and procedures as stated in the physical therapy and occupational therapy plan of care.

Job Requirements:

Current PT or OT MA Licensure

1 to 2 years of Homecare experience preferred

Must currently hold a valid Driver’s License

Excellent communication verbal and written skills.

Excellent time management and interpersonal skills

Must be detail-oriented and a team-player

Ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

On Call Homecare/Hospice Registered Nurse 

Job Summary:  Weekly schedule will be Monday through Friday from 4:30pm to 8:00am. The evenings to be covered are Monday night through Friday night ending at 8:00am Saturday morning. The right candidate will provide on-call services for agency home health and hospice patients as received through the answering service. Triage calls and determine if a visit is needed to be made after discussion with patient or patient family. Report to scheduled evening/night visits or admissions. Keep ongoing log (as provided by Hospice Director) of all patient telephone calls and visits made during on call from 4:30pm to 8:00am. This is considered a weekly position and will be paid at a base weekly rate (minimum $387.50/week) plus differential for triaging calls and/or making home visits.

Job Requirements:  Current RN MA Licensure, Hospice experience, Home Health experience, valid driver's license, ability to learn and be proficient with required documentation software using agency electronic device.

Registered Dietician – Per-diem/Per-visit

Job Summary:  Provides home visits for nutritional assessment consultation and medical nutrition therapy under the care of a physician. Provides patient and family education regarding nutrition for wellness and in disease management.

Job Requirements:  BS in related field. Current licensure as registered dietitian in the state of Massachusetts.  Five or more years of professional experience in nutrition. Minimum of one year of health experience. Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages, especially the geriatric population.

Home Health Speech Language Pathologist – Per-Diem

Job Summary:  Evaluates plans and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, and language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.

Job Requirements:  A person who has a master’s or doctoral degree in speech language pathology, and is licensed as a speech-language pathologist by the state of MA. One plus years of experience in Home Care preferred.

Intake Nurse Coordinator – Full time (30 hrs. per week)

Job Summary:  Accepts, screens, coordinates and processes referrals for home health, hospice and community health services.  Maintains a positive, respectful attitude; demonstrates excellent customer service.  Accepts calls from all referral sources identifying service needs or community information.  Assists referral sources in evaluating service needs of client/family.  Explains intake and assessment process to client/family/referral source and interprets scope of services that are needed and the availability of those services.  Interprets Medicare, Medicaid and other third-party regulations to insure appropriate referrals to Agency.  Interprets Agency policy and capacity to meet requests for services.  Processes detailed information from referral sources/client/family/referral source prior to initial home assessment to enhance intervention.  Documents specifics on EMR (electronic medical record), e.g. wound care, teaching needs, IV medications, therapy needs, etc.

Job Requirements:  RN or LPN MA license (RN is highly preferred).  Two years of clinical RN or LPN experience in home health, med surg, subacute or rehab facility required.  1-2 years of related experience in admissions or intake office and ICD-10 coding strongly preferred.  Current knowledge of reimbursement for home health care by Medicare, Medicaid, private insurances and managed care organizations a plus.  Hospice experience a plus.  Positive, respectful attitude and excellent customer service skills.  Above average communication and organizational skills.  Demonstrate knowledge and experience of computer literacy (EMR, Word, Outlook).  Demonstrated knowledge, experience and strengths in telephone answering and customer service skills required.  Must be able to work Monday through Friday from either 11:30 am to 5:30 pm or 12:00 pm to 6:00 pm with occasional weekend and holiday coverage.

Homecare RN – Full-time (32 or 40 hrs. per week)

Job Summary:  As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor. The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients, provide professional in-home nursing care to clients, assess and evaluate client's status by reviewing and implementing the plan of care and regularly re-evaluating the client's needs, administer medications and treatments as prescribed by the physician. Observe client for changes in condition and discuss changes with the client, caregiver, and attending physician.

Job Requirements:  Current RN MA Licensure, 2 years home health nursing experience preferred (not required), 2 years of medical/surgical, sub-acute preferred (not required), currently holds a valid driver’s license, ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.
