Homecare and Hospice Social Worker (LICSW)

Responsibilities

Assist and counsel patients and families with financial, social and emotional concerns primarily in hospice and homecare programs.

Provides social work services in accordance with the physician’s plan of treatment.

Assists the physician and other members of the home health team to understand the significance of social and emotional factors related to the patient’s care.

Assesses and reassesses social and emotional factors in order to help the patient and family to cope with problems in daily living and to help them follow medical recommendations to maximize the patient’s health condition.

Counsels patients and families concerning the facilitation of the medical plan of treatment.

Recognizes and utilizes community and family resources to assist the patient in long-term plans for life in the community or to learn to live within the patient’s disability.

Develops the plan of care with the primary nurse, physician, patient and family to deal with personal, financial and environmental difficulties that may interfere with the health and wellness continuum. Participates in discharge planning.

Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation within 24 hours of visit.

Requirements: Bachelor or Master's degree from a school of social work accredited by the Council on Social Work Education.

**Licensed as an independent clinical social worker (LICSW) in MA.

Minimum of one to three years of experience in social work, preferably in a home care agency, hospice, hospital, outpatient clinic, rehabilitation center or mental health program.

Required experience: 1 year of Hospice/Homecare

Intake Coordinator – LPN or RN – 30 hrs./week

Accepts, screens, coordinates and processes referrals to the NNS&H for home health, hospice and community health services. Maintains a positive, respectful attitude; demonstrates excellent customer service. Accepts calls from all referral sources identifying service needs or community information. Assists referral sources in evaluating service needs of client/family. Explains intake and assessment process to client/family/referral source and interprets scope of services that are needed and the availability of those services. Interprets Medicare, Medicaid and other third party regulations to insure appropriate referrals to Agency Interprets Agency policy and capacity to meet requests for services. Processes detailed information from referral sources/client/family/referral source prior to initial home assessment to enhance intervention. Documents specifics on EMR (electronic medical record), e.g., wound care, teaching needs, IV medications, therapy needs, etc. Requirements: RN or LPN MA licensure. Two years of clinical RN or LPN experience in home health, med surg, subacute or rehab facility required.1-2 years of related experience in admissions or intake office and ICD-10 coding strongly preferred. Current knowledge of reimbursement for home health care by Medicare, Medicaid, private insurances and managed care organizations a plus. Hospice experience a plus. Positive, respectful attitude and excellent customer service skills. Above average communication and organizational skills. Able to work 8:00 am to 4:30 pm and 10:00 am to 6:30 pm rotating shifts Monday through Friday with some flexibility as well as occasional weekends. Demonstrate knowledge and experience of computer literacy (EMR, Word, Outlook).Demonstrated knowledge, experience and strengths in telephone answering and customer service skills required.

Per Diem Home Health Aides

Job Duties: Provides personal care services, such as: • reading and recording temperature, pulse and respiration • maintaining a clean, safe and healthy environment •bathing • caring for nails and skin • oral hygiene • assists with self-administered medications • assists with Hoyer lift and other durable medical equipment. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning and doing household chores. Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home. Observes, reports and documents changes in patient status or expectations of service promptly to a supervisor or professional.

Requirements: Current Certification as MA CNA or CHHA (one to two years’ experience required as a CNA and 2 years’ experience as a Certified Home Health Aide. Must have own reliable transportation. Must have valid driver’s license.

Per-diem Home Care/Hospice Licensed Practical Nurse

Job Responsibilities:

Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies.

Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case.

Assists clients in learning appropriate self-care techniques.

Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.

Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.

Assists the physician and registered nurse in performing specialized procedures for which competency has been established.

Prepares equipment and materials for treatments observing aseptic technique as required.

Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.

Attends client care conferences, meets with the quality improvement reviewer weekly and communicates regularly with the registered nurse.

Communicates current health status and needs of clients to appropriate health professionals.

Requirements:

Successful completion of a certified LPN nursing program.

Licensed to practice as a practical/vocational nurse in the state.

Experience in Homecare preferred

Skills and Abilities:

Must currently hold a valid Driver’s License.

Must be flexible to make home care visits as agency needs.

Excellent communication skills in both verbal and written

Excellent time management and interpersonal skills

Must be detail-oriented and a team-player

Ability to learn and be proficient with required documentation software using agency electronic device.

Home Care – Hospice Registered Nurse – Per Diem

Job Summary: As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor. The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients.

Responsibilities: Provides services requiring basic, substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician and makes the initial evaluation visit to the patient. Evaluates, and regularly reevaluates, the nursing needs of the patient. Initiates, develops, implements and makes necessary revisions to the patient’s plan of care. Assesses the patient’s continual care needs. Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs. Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in the patient’s condition. Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Coordinates the total plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients.

Job Requirements:

Current RN MA Licensure.

2 years home health nursing experience preferred

2 years of medical/surgical, subacute preferred.

Currently holds a valid driver’s license.

Must be flexible to make home care visits as agency needs.

Required on call rotation for weekends, evenings and Holidays.

Ability to learn and be proficient with required documentation software using agency electronic device.

Required education: Associate

Required experience: Home Health Nursing: 2 years

Required licenses or certifications: Registered Nurse (RN), Driver's License
