Physical Therapist Assistant

The Physical Therapist carries out treatment, implementation and documentation of patient status for home health patients under the direction of the registered physical therapist.

Job Requirements:

  • Current MA Licensure as a Physical Therapy Assistant

  • Graduate of a two-year, college-level program approved by the American Physical Therapy Association.

  • Minimum of 2 years of experience as PTA in a hospital, outpatient rehab facility or sub-acute skilled nursing facility preferred.

  • Currently holds a valid driver’s license.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekends, evenings and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Home Care RN Case Manager - full and part time

As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor.  The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients.

Job Requirements:

  • Current RN MA Licensure.

  • Minimum 2 years home health nursing experience required, and 2 years of medical/surgical, subacute preferred.

  • Currently holds a valid driver’s license.

  • Must be flexible to make home care visits as agency needs.

  • Required on call rotation for weekends, evenings and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Hospice RN Case Manager - full or part time

As a member of the interdisciplinary team, the Hospice RN works under the direct supervision of the Clinical Manager.  The Hospice RN will coordinate nursing assessment, planning and care in order to maximize the comfort and health of patients and their families. S/he is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the primary care physician’s orders and the plan of care.

Job Requirements:

  • Current RN MA Licensure.

  • Minimum of 2 years of Hospice and Homecare experience required, 2 years medical/surgical, subacute experience preferred.

  • Currently holds a valid driver’s license.

Homecare and Hospice Social Worker (LICSW) Part-time to Full -time

Assist and counsel patients and families with financial, social and emotional concerns primarily in hospice and homecare programs. 

Job Requirements:

  • Bachelors or Master’s degree from a school of social work accredited by the Council on Social Work Education.

  • Licensed as an independent clinical social worker (LICSW) in MA.

  • Minimum of one to three years of experience in social work, preferably in a home care agency, hospice, hospital, outpatient clinic, rehabilitation center or mental health program.

Clerk, Environmental Department

We currently are seeking a part time Clerk for our Environmental division to support the administrative staff in the operation of the office. Hours are three days a week: M, W and F 9:00 am to 3:00 pm.

Job Responsibilities:

  • Represents the agency as initial contact (by telephone or in person) in a positive manner.

  • Responds to agency telephone calls from public, staff and vendors triaging calls to appropriate staff, takes messages and pages staff as needed.  Prepares and processes mail. Processes and distributes all faxes.

  • Performs word processing projects.   Performs copying assignments as needed.

  • Maintains office filing system, filing, research, data entry and processes licensing paperwork.

Job Requirements:

  • High School graduate or Equivalent.

  • Minimum of one year of office experience to included operation of copiers, faxes, postage machines, etc.

  • Word processing experience preferred as well as six months to one year of computer experience.

  • Excellent communication skills.

  • Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages.

Community Health Social Worker (LICSW)

We currently are seeking a full time Community Health Social Worker (LICSW) to join our team of dedicated and talented professionals serving patients and our member communities.

Job Responsibilities:

  • Provides team support to community health nursing team to enhance wellness and safety of populations served by NABH. Community Health Social Worker offers outreach and education to these communities that encompass a wide variety of community health issues.

  • Assists the community health team and other involved agencies to understand the significance of social and emotional factors related to the client needs.

  • Assesses and reassesses social and emotional factors in order to help the client and family to cope with problems in daily living and to help them follow medical recommendations to maximize the client’s health and wellness.

  • Counsels clients and families concerning the facilitation of the plan of treatment in compliance with health and wellness standards of public health philosophy.

  • Recognizes and utilizes community and family resources to assist the client in long-term plans for life in the community or to learn to live within the client’s disability.

  • Identifies and collaborates with appropriate agencies to meet the needs of client goals.

  • Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies.

  • Develops and presents programs and facilitates informational discussions/forums.

  • Keeps abreast of social work trends and remains competent in issues affecting public health.  Participates in in-service programs to keep up with licensure.

  • Provides outreach and education to serviced communities regarding community health issues.

  • Leads community initiatives to support intervention of community health concerns affecting NABH towns.

  • Follows mandated reporting laws when necessary re: legal and safety issues to ensure client safety and community wellness.

Job Requirements:

  • Master’s degree from a school of social work accredited by the Council on Social Work Education.

  • Licensed as a social worker in the state or meets all education and experience requirements for licensure in the state of and lacks only the examination. LICSW preferred.

  • Minimum of one to three years of experience in social work, preferably in a home setting, or outreach, case management, outpatient clinic, rehabilitation center or mental health program.

  • Knowledge and Abilities:

  • Interest in the area of hoarding preferred and ability to work in challenging home environments.

  • Strong working knowledge of local resources.

  • Demonstrated ability to collaborate with local authorities/ agencies.

  • Strong communication skills and working knowledge of Microsoft Office Suite.
