Hospice RN – Sign on bonus $2500.00 (Full-time status)

Job Summary:  As a member of the interdisciplinary team, the Hospice RN works under the direct supervision of the Hospice and Palliative Care Director. The Hospice RN will coordinate nursing assessment, planning and care in order to maximize the comfort and health of patients and their families. S/he is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the primary care physician’s orders and the plan of care. Assumes primary responsibility for nursing care to patients and support to their families, including assessment, implementation, and evaluation of the plan of care in all settings. Communicates with the Medical Director, other interdisciplinary team members, attending physicians, and other agency personnel for coordination of care and utilization of resources for patient and family. Attends meetings, as a member of the Interdisciplinary Team, and facilitates communication of issues for full staff discussion, consultation, knowledge, and support in updating the plan of care. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family end of life care. Maintains up-to-date electronic patient records so that problems, plans, actions and goals are accurately and clearly stated and that changes are reflected as they occur. Provides appropriate support at time of death and participates in the initial bereavement plan of care.

Job Requirements: Current RN MA Licensure, Minimum of 2 years of Hospice and Homecare experience required, 2 years medical/surgical, sub-acute experience preferred, currently holds a valid driver’s license. Must be flexible to make home care visits as agency needs, required on call rotation for weekends and evenings, ability to learn and be proficient with required documentation software using agency electronic device.

Homecare RN – Per-Visit 

Job Summary:  As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor. The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients, provide professional in-home nursing care to clients, assess and evaluate client's status by reviewing and implementing the plan of care and regularly re-evaluating the client's needs, administer medications and treatments as prescribed by the physician. Observe client for changes in condition and discuss changes with the client, caregiver, and attending physician

Job Requirements: Current RN MA Licensure, 2 years home health nursing experience preferred (not required), 2 years of medical/surgical, sub-acute preferred (not required), currently holds a valid driver’s license, ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

Physical Therapist – Per diem

Job Summary: As a member of the interdisciplinary team, the Homecare Physical Therapist will coordinate therapy assessment, planning and care in order to maximize the health of patients. The Homecare Physical Therapist works closely with Physical Therapist Assistant to carry out plan of care in the home care setting.

Provide skilled physical therapy in accordance with the physician’s plan of care, provide a one-on-one patient experience, treating patients to develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.

Job Requirements: Current RPT MA Licensure, 1 to 2 years of Homecare experience preferred, must currently hold a valid Driver’s License, must be flexible to make home care visits as agency needs, required on call rotation for weekend and holidays, excellent communication skills in both verbal and written, excellent time management and interpersonal skills, must be detail-oriented and a team-player, ability to learn and be proficient with required documentation software using agency electronic device. 

On Call Homecare/Hospice Registered Nurse

Job Summary: Weekly schedule will be Monday through Friday from 4:30pm to 8:00am. The evenings to be covered are Monday night through Wednesday night ending at 8:00am Thursday morning. The right candidate will provide on-call services for agency home health and hospice patients as received through the answering service. Triage calls and determine if visit is needed to be made after discussion with patient or patient family. Report to scheduled evening/night visits or admissions. Keep ongoing log (as provided by Hospice Director) of all patient telephone calls and visits made during on call from 4:30pm to 8:00am. This is considered a weekly position and will be paid at a base weekly rate (minimum $240.00/week) plus differential for triaging calls and/or making home visits.

Job Requirements:

Current RN MA Licensure, Hospice experience, Home Health experience, valid driver's license, ability to learn and be proficient with required documentation software using agency electronic device.

Office Support Associate

Job Summary:

Manage Medical Records workflow by prioritizing tasks, ensuring that deadlines are met and work is completed correctly, assist in cross training other support staff and new hires in Medical Records, department based support for Scheduling and Intake, generate memos, letters and emails when appropriate, assume responsibility for maintenance of office equipment, including copy/fax machine and postage machine, maintain office and medical supplies by checking inventory and order items, respond to questions and requests for information for managers and field staff, answer incoming calls and assume other receptionist duties when needed, seeking a reliable, organized Office Support Associate to support the day-to-day administrative tasks. The successful applicant will have a positive attitude, a desire to work as efficiently as possible and excellent communication skills. This job may include some weekend and holiday rotation. We're looking for someone who can operate effectively with little or no supervision and who can manage multiple tasks at once without becoming overwhelmed. The ideal candidate for this job is resourceful, a good problem solver and organized. Assuring a steady completion of workload in a timely manner is key to success in this position.

Job Requirements:

High School Degree, Associates Degree in Business Administration or a Medical Secretary Certificate preferred. Home Health/Hospice/Medical Office experience, experience in Scheduling is preferred, Proficiency in MS Office with expertise in Microsoft Outlook, Word, PowerPoint and Excel, Adobe Standard/Professional experience, Experience working with an Electronic Health Record; Home Care Home Base preferred, ability to analyze and revise office procedures to improve efficiency, Detail oriented and comfortable working in a fast-paced office environment, Exceptional communication skills, Superior organization skills and dedication to completing projects in a timely manner. 

Marketing and Community Relations Liaison

Job Summary:  Marketing/Community Liaison professional to perform activities associated with customer relations and marketing of the Agency to establish solid referral sources and drive home care and hospice admissions in our service area. Promote and build strong customer relations, primarily with physician groups including cold calling and setting appointments to explain new and on-going programs to obtain new business. Develop, implement and monitor sales and marketing plans and action programs to increase and maintain current relationships and referrals and secures feedback from referral sources. Liaison to hospitals, rehab facilities, skilled nursing and assisted living facilities, physician offices, clinics, etc. Coordinate, plan and prepare the transition of patient care to the home from the referral source. Plan, direct and develop communication of agency accomplishments and agendas. Publish newsletter regularly for the public and maintain accuracy of mailing lists. Develop advertising materials and ensures NNS&H is visible in the community. Develop and execute social media strategy and maintain agency website.

Job Requirements: Current MA RN Licensure, Bachelor’s degree with emphasis in marketing with two years relevant sales, marketing communications or public relations experience. Two to three years’ experience in health care or clinical experience as RN preferred. Excellent written and verbal communication skills. Computer experience with various word processing, database, spreadsheet applications and other graphic and presentation software is necessary. Able to overcome resistance and objections by referrals sources. Proven track record in producing new business. Ability to attend health fairs, clinics, meetings with facilities, etc. outside normal workday, for example, Saturdays or evenings.
