Hospice RN – Sign on bonus $2500.00 (Full-time status)

Responsibilities:  As a member of the interdisciplinary team, the Hospice RN works under the direct supervision of the Hospice and Palliative Care Director. The Hospice RN will coordinate nursing assessment, planning and care in order to maximize the comfort and health of patients and their families. S/he is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the primary care physician’s orders and the plan of care. Assumes primary responsibility for nursing care to patients and support to their families, including assessment, implementation, and evaluation of the plan of care in all settings. Communicates with the Medical Director, other interdisciplinary team members, attending physicians, and other agency personnel for coordination of care and utilization of resources for patient and family. Attends meetings, as a member of the Interdisciplinary Team, and facilitates communication of issues for full staff discussion, consultation, knowledge, and support in updating the plan of care. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family end of life care. Maintains up-to-date electronic patient records so that problems, plans, actions and goals are accurately and clearly stated and that changes are reflected as they occur. Provides appropriate support at time of death and participates in the initial bereavement plan of care.

Job Requirements:

  • Current RN MA Licensure.
  • Minimum of 2 years of Hospice and Homecare experience required
  • 2 years medical/surgical, sub-acute experience preferred
  • Currently holds a valid driver’s license
  • Must be flexible to make home care visits as agency needs
  • Required on call rotation for weekends and evenings - Full-time Employee's
  • Ability to learn and be proficient with required documentation software using agency electronic device.

Homecare RN – Fulltime and Per-Visit 

Job Summary:  As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor. The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients.

Responsibilities:

  • Provide professional in home nursing care to clients

  • Assess and evaluate client's status by reviewing and implementing the plan of care and regularly re-evaluating the client's needs

  • Administer medications and treatments as prescribed by the physician

  • Observe client for changes in condition and discuss changes with the client, caregiver, and attending physician

Job Requirements:

  • Current RN MA Licensure.

  • 2 years home health nursing experience preferred (not required)

  • 2 years of medical/surgical, sub-acute preferred (not required)

  • Currently holds a valid driver’s license.

  • Ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.

Physical Therapy Assistant - Fulltime

The Physical Therapist Assistant carries out treatment, implementation and documentation of patient status for home health patients under the direction of the registered physical therapist.

Job Requirements:

  • Current MA Licensure as a Physical Therapy Assistant

  • Graduate of a two-year, college-level program approved by the American Physical Therapy Association.

  • Minimum of 2 years of experience as PTA in a hospital, outpatient rehab facility or sub-acute skilled nursing facility preferred.

  • Currently holds a valid driver’s license.

  • Must be flexible to make home care visits at agency needs.

  • Required on call rotation for weekends, evenings and Holidays.

  • Ability to learn and be proficient with required documentation software using agency electronic device.

Certified Home Health Aide and/or Certified Nursing Assistant

Responsibilities:  Provides personal care services such as • reading and recording temperature, pulse and respiration • maintaining a clean, safe and healthy environment • bathing • caring for nails and skin • oral hygiene • assists with self-administered medications • assists with Hoyer lift and other durable medical equipment. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning and doing household chores. Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home. Observes, reports and documents changes in patient status or expectations of service promptly to a supervisor or professional.

Requirements:  Current Certification as MA CNA and/or CHHA (one to two years’ experience required as a CNA and/or 2 years’ experience as a Certified Home Health Aide). Must have own reliable transportation. Must have valid drivers license.
