The Nashoba Associated Boards of Health Community Health Team's mission is to provide high quality Public Health Nursing to promote wellness across the lifespan in our 16 member communities. We offer a health promotion program which includes monthly clinics for health screenings, information and resource referrals, education and outreach, and home visits as needed. The nursing team also works closely with the Massachusetts Department of Public Health to focus on disease prevention. We aim to prevent disease and protect the health of our communities by providing immunization clinics, regular communicable disease investigation and follow-up and by responding to disease outbreaks. Our staff is considered the "Town Nurse" for our communities.
The Town Nurse is available for consultation M-F 8 AM - 4:30 PM and can be reached by calling 978-772-3335 or 1-800-427-9762, ext. 340.
As a town nurse, Tamara enjoys getting to know each town to best serve and meet the needs of the community. She invites you to come and see her at the well-adult clinics where you may have your blood pressure checked or just talk about concerns. She is also available for home visits to make general assessments & help with coordinating care and make referrals to community and other supportive resources. As a nurse working for the local boards of health, it is also her job to investigate and follow-up incidences of reportable, communicable/infectious diseases as required by the Massachusetts Department of Public Health for your town. As you might imagine, no two days are alike and this keeps her work very interesting.
