Nashoba Health Equity Partnership (HEP):

Under the regional Community Health Improvement Plan (CHIP) and in partnership with the Health Equity Partnership of North Central MA (CHNA9), the Nashoba Associated Boards of Health leads a local micro-coalition, the Nashoba Health Equity Partnership (HEP), a permanent structure capable of advancing local community-led public health initiatives and responding to local needs and opportunities.


The Nashoba HEP was launched through a grant from the Health Foundation of Central Massachusetts, as part of the Commonwealth’s Advancing Health Equity in Massachusetts (AHEM) initiative. This micro-coalition allows us to focus on hyper-local concerns and resources, while benefiting from the support of a larger regional umbrella coalition.


The Nashoba HEP staff runs quarterly coalition-wide meetings along with regular working group meetings and other events. Each of the four working groups was formed in response to a particular priority area in the recently completed Nashoba Community Health Needs Assessment:


1. Social Isolation, Aging Population, & Digital Equity

2. Healthcare Access

3. Youth Mental Health

4. Holistic Community Planning
